But an increasing amount of research shows the chemicals in e-cigarettes start to inflict damage right where they enter the body: your mouth. Because e-cigarettes are a recent phenomenon, said Dr Crystal Stinson, assistant professor at Texas A&M College of Dentistry in Dallas, "Studies on their impact are really new. But now we have a solid amount of evidence that shows the link between e-cigarettes and poor oral health." Nicotine, whether smoked or vaped, restricts blood flow to the gums, which can contribute to periodontal disease. The fluid in e-cigarettes, which can include propylene glycol, benzene, formaldehyde and other chemicals, only increases the risks. Irreversible issues A study published earlier this year in the journal iScience showed that 43% of people using e-cigarettes had gum disease and oral infections. That figure was higher among smokers – 73% – but only 28% among people who neither smoked nor vaped. "The oral cavity is really resilient tissue that heals faster than other parts of the body," Stinson said. "But we also know that when you repeatedly traumatise it, that's when you end up having issues that are irreversible." Vaping e-cigarettes could increase the risk of oral disease, study finds

Carried out by researchers at The Ohio State University, the new study looked at a group of 123 people with no signs of oral disease. The group included 25 smokers, 25 non-smokers, 20 e-cigarette users, 25 former tobacco smokers who used e-cigarettes and 28 people who smoked both tobacco cigarettes and e-cigarettes. The team collected plaque samples taken from under the gums of the participants to analyze the bacteria in this part of the mouth; bacteria here is the last line of defense against disease as it is the least likely to be disrupted by environmental changes in the mouth, such as food, toothpaste and tobacco. The researchers then carried out DNA deep sequencing of the bacteria’s genomes to identify what types of microbes were living in participants’ mouths and what their functions were. The findings, published in the journal Science Advances, showed that although the e-cigarette users didn’t have signs of active disease, their oral bacteria composition was similar to that of people with severe periodontitis, a severe gum infection that can lead to health problems such as tooth loss, and, if left untreated, is a risk factor for serious conditions such as heart and lung disease. The impact of artificial intelligence (AI) will increase in the future. AI is already at work in hospitals to diagnose cancer and anticipate trends in health care. AI will have a significant effect in the dental profession on a daily basis, from evaluating images for pathology, to prosthetics and systemic care, among many others. AI promises to increase efficiency in dental practice by facilitating faster diagnosis, predictive analytics, and autocharting. Other areas of research and development include gene therapy and stem cells. Research is underway using gene therapy to restore salivary function in patients who have undergone radiation treatment, which could be a tremendous improvement in health for these individuals. The negative impact of dry mouth extends far beyond the discomfort associated with inadequate or complete lack of saliva. Bacteria linked to periodontitis may play role in onset of cancer

Source: en.brinkwire.com Author: press release The bacteria that cause periodontitis, a disease affecting the tissues surrounding the teeth, seems to play a part also in the onset of pancreatic cancer, say the researchers at the University of Helsinki and the Helsinki University Hospital, Finland, and the Karolinska Institutet, Sweden. The researchers have investigated the role of bacteria causing periodontitis, an inflammation of the tissues surrounding the teeth, in the development of oral cancers and certain other cancers, as well as the link between periodontitis and cancer mortality on the population level. The study, published in the British Journal of Cancer, has for the first time proven the existence of a mechanism on the molecular level through which the bacteria associated with periodontitis, Treponema denticola (Td), may also have an effect on the onset of cancer. Researchers found that the primary virulence factor of the Td bacteria, the Td-CTLP proteinase (an enzyme), occurs also in malignant tumors of the gastrointestinal tract, for example, in pancreatic cancer. According to another study finding, the CTLP enzyme has the ability to activate the enzymes that cancer cells use to invade healthy tissue (pro-MMP-8 and -9). At the same time, CTLP also diminished the effectiveness of the immune system by, for example, inactivating molecules known as enzyme inhibitors. In another study, published in the International Journal of Cancer, it was proven that on the population level, periodontitis is clearly linked with cancer mortality. Males have a greater risk of developing the disease compared to females.2 Black males in particular are amongst the highest at-risk group for developing oral carcinoma.2 Oral carcinoma typically develops after the age of 50, with the majority of cases occurring between the ages of 60 and 70.2 When initially diagnosed with oral carcinoma, more than 50% of people will have metastases.3 The most common causes of oral carcinoma are related to tobacco use and alcohol consumption.4 In fact, 75% of all cases of oral carcinoma may be caused by the combination of tobacco and alcohol use.4 However, it has also been extrapolated that chronic trauma to the oral mucosa, such as in the case of ill-fitting dentures or the consumption of high-temperature foods, is a leading modifiable risk factor for oral carcinoma.1,5 Dietary deficiencies of vitamins A, C, E, selenium, and folates may also contribute to the development of malignant cancerous lesions in the oral cavity.6 While cases of oral carcinoma have decreased over the last few years in the United States, oropharyngeal cancer is increasing in incidence.4 The rise in cases of oropharyngeal cancer may be related to viral and infectious diseases; however, the mechanisms are largely unclear. Like many cancer patients, Jennifer Giesel has side effects from treatment. There’s the neuropathy in her hands, a holdover from chemo. There’s jaw stiffness from her multiple surgeries: an emergency intubation when she couldn’t breathe due to the golf ball-sized tumor on her larynx and two follow-up surgeries to remove the cancer. And then there’s hypothyroidism and xerostomia, or dry mouth, a result of the 35 radiation treatments that beat back the cancer but destroyed her salivary glands and thyroid. “I went to my primary care doctor a couple of times and mentioned the side effects,” said the 41-year-old laryngeal cancer patient from Cleveland, who was diagnosed two years ago. “She was great but she didn’t seem too knowledgeable about what I was telling her. She was like, ‘Oh really?’ It was more like she was learning from me.” Patients like Giesel should have an easier time communicating their unique treatment side effects to health care providers with the recent release of new head-and-neck cancer survivorship guidelines. Periodontitis increases risk of oral leukoplakia

Source: www.drbicuspid.com Author: Donna Domino, Features Editor Periodontitis increases the risk of developing oral leukoplakia and mucosal lesions that are predisposed to become oral cancer, according to a study in Oral Oncology (September 2012, Vol. 48:9, pp. 859-863). The findings provide clues into the complex relationship between systemic and local disease, noted the study authors from the University of Greifswald in Germany. The development of oral cancer proceeds through discrete molecular changes that are acquired from loss of genomic integrity after continued exposure to environmental risk factors. It is preceded in the majority of cases by clinically evident, potentially malignant oral disorders, the most common of which is leukoplakia, the researchers noted. Leukoplakia is an asymptomatic lesion in the oral mucosa. Oral cancer -- especially oral squamous cell carcinoma -- often develops out of these lesions, they added. Studies have shown that as many as 18% of oral premalignant lesions will develop into oral cancer. In addition, periodontal sites are often involved in proliferative types of leukoplakia. The oral cancer rate attributed to leukoplakia is between six and 29 per 100,000, according to the authors. Smoking and drinking alcohol are the main risk factors for this disease, but acute infections in the oral cavity may contribute to the risk. Study links gum disease, HPV-status of head and neck cancer

Furthermore, according to a new UB study just published in the Archives of Otolaryngology—Head & Neck Surgery, a JAMA publication, gum disease is associated with increased odds of tumors being HPV-positive. Primary investigator Mine Tezal, assistant professor of oral biology in the School of Dental Medicine, and a team of scientists from UB evaluated data from 124 patients diagnosed with primary head and neck squamous cell carcinoma (HNSCC) between 1999 and 2007. “The aim of the study was to test the presence of periodontitis, a persistent inflammatory process and HPV-status of HNSCC,” Tezal says. Of the 124 tumor samples Tezal and her team studied, 50 were positive for HPV-16 DNA and that subjects with HPV-positive tumors had a significantly higher severity of periodontitis when compared to subjects with HPV-negative tumors. According to the National Cancer Institute, there has been a steady increase in the prevalence of oropharyngeal cancers in the U.S. since 1973. This is despite the significant decline in tobacco use since 1965, a year after the U.S. Surgeon General issued the landmark Report on Smoking and Health. Emerging indications: antioxidants for periodontal disease

Allen, DDS, PhD Since approximately 10 to 15 percent of adults worldwide suffer from periodontitis at one time or another(1), oral health professionals are constantly challenged with treating patients for existing conditions and helping them prevent future occurrences. Root planing, scaling, and in severe cases, surgical intervention are part of the standard treatment for periodontitis, and antibiotics are used for infection control. However, in recent years, dental health professionals have honed in on the inflammation that accompanies periodontitis. Research shows that inflammation in the oral tissues—especially that associated with periodontitis—can be a factor in chronic illness such as heart and vascular disease, diabetes, arthritis, Alzheimer’s, pregnancy complications, and a growing list of other conditions. The real culprit with inflammation is oxidative stress, a disturbance in the balance of oxidants and antioxidants. Oxidative stress is the result of overproduction of free radicals, unstable molecules that attack tissue cells by “stealing” electrons from other molecules. Although infection is a major trigger for inflammation and oxidative stress, there are numerous other causes, such as poor diet, alcohol consumption and nicotine use or chemical pollutants. In oral tissues oxidative stress can result from dental procedures and from materials used for bleaching, composite fillings, implants, crowns, veneers, and so on. Scientific literature supports safety of essential oil mouthrinses

An antimicrobial mouthrinse where the active ingredient is the fixed combination of four essential oils (EO), thymol, menthol, eucalyptol and methyl salicylate in a hydroalcohol solution (pharmaceutical grade ethanol functions as a solvent, a vehicle for the active ingredients and as a preservative in a mouthrinse), has been proven clinically effective and safe in over 50 clinical trials, including 6-month trials with plaque reductions up to 70.0%(1) and gingivitis reductions up to 36%(1,2) compared with negative control. Sharma, et al, showed the clinically significant incremental benefit of including an EO antimicrobial mouthrinse in a daily mechanical oral regimen of brushing and flossing.(3) Mechanical oral hygiene techniques generally focus on the teeth, but teeth comprise only about 25% of the surface area of the mouth.
