Medicare expands coverage of tobacco cessation

"Most Medicare beneficiaries want to quit their tobacco use," Health and Human Services Department (HHS) Secretary Kathleen Sebelius said in a statement announcing the move. "Now, [they] can get the help they need." Under previous rules, Medicare covered tobacco-related counseling only for beneficiaries already suffering from a tobacco-related disease. Under the new policy, Medicare will cover as many as two tobacco-cessation counseling tries each year, including as many as four individual sessions per attempt. The move is the latest in a string of White House efforts to shift the nation's healthcare system toward prevention, in lieu of simply treating diseases after they've developed. If successful, the new tobacco policy could pay dividends. Of the 46 million Americans estimated to smoke, about 4.5 million are seniors older than 65, HHS says. And nearly 1 million more smokers are younger than 65, but eligible for Medicare benefits. They aren't cheap. Tobacco-related diseases are estimated to cost Medicare about $800 billion between 1995 and 2015. Donald Berwick, head of the Centers for Medicare and Medicaid Services, said the expansion lends seniors valuable help "to avoid the painful — and often deadly — consequences of tobacco use." The change affects Medicare Parts A and B — hospital care and physician services — but not Part D, which already covers smoking-cessation drugs for all beneficiaries.

New tobacco policy likely to help check smoking among teenagers

Because of this, the number of youngsters who start smoking at a very early age has decreased significantly. Researchers at University College London interviewed as many as 1,000 people aged 16-17 before and after the age limit was changed in the month of October in 2007. It was found that previous to October 2007, 24% of teenagers said that they smoked regularly. After the implementation of the new policy, the number reduced to 17%. The percentage of those above 18 years of age remained the same. It is worth mentioning that 8 out of every 10 smokers acquired this habit before they reached 19 years of age and diseases like mouth cancer and other problems related to tobacco are increasing rapidly. Researchers believe that the new tobacco law will certainly bring down the number of teenagers who want to go for smoking. According to Jenny Fidler, one of the researchers of the study, "The new law looks to have helped reduce smoking prevalence among younger age groups". He further added that the new law is for the betterment of the young generation of the country. He also said that new law will certainly clamp down on those teenagers who start smoking before 19 years.

Oral health experts and the Foundation are advising the public to regularly check their mouths after news broke this week of actor Michael Douglas being diagnosed with oral cancer. The Academy Award winner has recently been diagnosed with a tumour in his throat, and now faces an eight–week cause of chemotherapy and radiotherapy. This high profile case has brought oral cancers into the lime light, and oral health experts are keen to make the public more aware of the key risk factors and early warning signs. Douglas quit smoking in 2006, after a long ‘half a packet a day’ habit. Yet, the possibility of developing oral cancer remains higher for ex–smokers than non–smokers for 20 years after quitting. Tobacco is considered to be the main cause of mouth cancer, with three in four cases being linked to smoking. Drinking in excess is also a known factor, with those who both smoke and drink to excess being up 30 times more likely to be at risk. HPV-positive oropharnygeal cancer has better prognosis than tobacco-induced cancer

The most recent supportive evidence comes from an analysis of a Phase III trial presented at the 2009 annual meeting of the American Society of Clinical Oncology. Our study showed that HPV status is as strong a predictor of outcome as cancer stage for patients with oropharyngeal cancers, even after considering other factors such as age and smoking history, said lead author Maura Gillison, MD, PhD, Professor of Hematology and Oncology, Epidemiology, and Otolaryngology at Ohio State University in Columbus. Dr. Gillison said that tumor HPV status should now be part of the routine workup of patients with oropharyngeal cancers. Oropharyngeal cancers are mainly attributable to chronic tobacco use and smoking, or to HPV infection. Retrospective analyses, meta-analysis, and small trials have suggested that HPV-positive oropharyngeal cancer is a distinct entity, and the present Phase III study provides the most compelling evidence, she said, because it is the largest study to date. It is not clear why HPV-associated oropharyngeal cancer has a better prognosis. In the trial, HPV-positive patients were younger, mostly Caucasian, and had improved performance status and smaller tumors. High-risk human papillomavirus in esophageal squamous cell carcinoma

Methods: To explore this issue, we analyzed HPV DNA prevalence and HPV types together with lifestyle factors, in relation to tumor stage and survival in a low-incidence population. Archived tumor samples from a nationwide cohort of 222 ESCC patients were tested for the presence of HPV DNA by PCR; positive samples were sequenced to determine HPV type, and p16INK4a status was assessed by immunohistochemistry. Results: Of 222 ESCC patients, 8 tested HPV positive (prevalence, 3.6%; 95% confidence interval, 1.1-6.1%), of which 6 were HPV-16 positive and 2 were HPV-35 positive. Four of the eight HPV-positive tumors overexpressed p16INK4a. None of 55 normal esophageal tissue samples from healthy participants had any detectable HPV. Although the numbers were low, it seemed that patients with HPV-positive ESCC tumors were younger than those with HPV-negative tumors (mean age, 60.8 versus 65.3 years, P = 0.18) and had higher body mass index (BMI) throughout life (mean current BMI of 25.1 for HPV positive, 22.2 for HPV negative, P = 0.08; mean BMI at 20 years of 25.8 for HPV positive, 22.1 for HPV negative, P = 0.003). New types of smokeless tobacco present growing risks for youth: products mistaken for candy

But as public health makes inroads, the tobacco industry is pushing back, offering new ways to deliver nicotine and hook lifelong customers. As nonsmoking ordinances sweep across the country, tobacco manufacturers are marketing new smokeless and spitless tobacco products, often selling them as complementary products to cigarettes — pushing the message that such “novel” tobacco products can deliver a nicotine fix whether smoking is allowed or not. Going by brand names such as Orbs, Snus and Taboka, the products are smokeless and spitless, often dissolving in a user’s mouth. For example, Marlboro Snus comes in a teabag-like pouch that a user puts between the cheek and gum and then discards after about 30 minutes. Camel Orbs look similar to small pieces of candy — almost like a Tic Tac — come in flavors such as “fresh” and “mellow,” and dissolve in a user’s mouth. Camel Sticks and Camel Strips also dissolve in a user’s mouth. A recent Camel Snus ad reads “Boldly Go Everywhere” and “Break Free.” While such products have yet to catch on in a significant way in the United States, public health advocates warn not to underestimate the influence of tobacco marketing, especially in regard to young people. "Camel Snus—extremely aggressive and the first to market in this new and growing category," a Virginia retailer said of the smokeless, spitless product. "'Fill the Fridge' is a good concept." Added a retailer from Iowa: "Snus continues to be a growth driver. Camel did a really good job out of the chute with its initial launch and now Marlboro has gotten into the game as well." Indeed, more than half of the 50-plus respondents cited the snus segment as a strong nascent player that is slowly gaining recognition and consumer acceptance. Congress approves bill curbing internet tobacco sales in victory for kids and taxpayers

Myers Voting 387 to 25, the U.S. House of Representatives today gave final congressional approval to the Prevent All Cigarette Trafficking (PACT) Act, legislation to curtail the growing sales of tax-evading, low-cost cigarettes and other tobacco products over the Internet and through the mail.  Passage of this legislation is a milestone in the fight to keep kids from smoking and prevent tax evasion that costs state and federal taxpayers billions each year. We applaud Rep. Anthony Weiner (D-NY), the chief House sponsor, and Sen. Herb Kohl (D-WI), the Senate sponsor, for their leadership and persistence in pursuing this legislation and winning its approval. The Senate unanimously approved the bill onMarch 11.  We look forward to President Obama continuing his strong leadership on tobacco control by signing the PACT Act into law. AACR recognizes one-year anniversary of tobacco law and the additional provisions taking effect today; calls for more research to combat tobacco epidemic

The law empowered the Food and Drug Administration (FDA) to regulate the marketing, advertising and manufacturing of tobacco products. Provisions that go into effect today include restrictions on youth access to tobacco products, enhanced warning labels on smokeless tobacco products, and a ban on the use of deceptive terms such as “light” or “mild.” “While most people know that cigarette smoking causes cancer, we need to do a far better job of educating the public about the risks of all tobacco products,” said Margaret Foti, Ph.D., M.D. (h.c.), chief executive officer of the AACR. “In fact, there is sufficient scientific evidence to causally link tobacco use to cancers at 18 different organ sites.
