New survey shows slow decline in youth smoking, troubling increase in smokeless tobacco use – congress, states must step up tobacco prevention efforts

Source: www.streetinsider.com Author: staff The 2009 Monitoring the Future Survey released today by the National Institute of Drug Abuse shows that the nation continues to make gradual progress in reducing youth smoking, but declines have slowed significantly compared to the dramatic gains early in the decade. In especially troubling news, the survey also finds that smokeless tobacco use has increased among 10th and 12th graders in recent years, a period during which tobacco companies have introduced a slew of new smokeless tobacco products and significantly increased marketing for smokeless tobacco. There is no question that we know how to dramatically reduce youth tobacco use. The use of proven strategies has caused smoking rates (the percentage who have smoked in the past 30 days) to decline by 69 percent among 8th graders, 57 percent among 10th graders and 45 percent among 12th graders since peaking in the mid-1990s. This is a remarkable public health success story. Before the recent increase, youth smokeless tobacco use also declined significantly from the mid-1990s to the early 2000s. However, the much slower progress in recent years is a clear warning to elected officials at all levels that they must resist complacency and redouble efforts to implement proven measures - rather than cutting tobacco prevention programs, as 34 states did this year. It is unacceptable to stand still or risk backsliding in the fight against the nation's number one preventable cause of death. Quitting cigarettes completely or switching to smokeless tobacco: do US data replicate the Swedish results?

Background: Swedish male smokers are more likely than female smokers to switch to smokeless tobacco (snus) and males’ smoking cessation rate is higher than that of females. These results have fuelled international debate over promoting smokeless tobacco for harm reduction. This study examines whether similar results emerge in the United States, one of few other western countries where smokeless tobacco has long been widely available. Methods: US data source: national sample in Tobacco Use Supplement to Current Population Survey, 2002, with 1-year follow-up in 2003. Analyses included adult self-respondents in this longitudinal sample (n = 15 056). Population-weighted rates of quitting smoking and switching to smokeless tobacco were computed for the 1-year period. Results: Among US men, few current smokers switched to smokeless tobacco (0.3% in 12 months). Few former smokers turned to smokeless tobacco (1.7%). Switching between cigarettes and smokeless tobacco, infrequent among current tobacco users (<4%), was more often from smokeless to smoking. Men quit smokeless tobacco at three times the rate of quitting cigarettes (38.8% vs 11.6%, p<0.001). Overall, US men have no advantage over women in quitting smoking (11.7% vs 12.4%, p = 0.65), even though men are far likelier to use smokeless tobacco. Maker of Camels buys cigarette replacement seller

The acquisition will let Reynolds offer products that can "reduce the risks of diseases and death caused by tobacco use," CEO Susan M. Ivey said in a statement. Niconovum AB's products are sold outside the U.S. under the Zonnic brand. They could help the maker of Camel cigarettes and Grizzly smokeless tobacco keep growing as tax increases, health concerns, smoking bans and social stigma cut into demand for cigarettes. The deal, which Reynolds expects to conclude by the end of the year, would be worth about $44 million. The Associated Press reported last month that the companies were in talks. Karl Olov Fagerstrom, an expert on smoking cessation and nicotine dependence, formed Niconovum in 2000, according to its Web site. Reynolds, which is based in Winston-Salem, N.C., said it will fund product development and testing required for Niconovum to enter markets outside of Sweden and Denmark. It intends to keep Niconovum's headquarters in Sweden and retain its leaders. Under the Camel brand, Reynolds has introduced moist smokeless tobacco and snus _ small pouches like tea bags that users stick between the cheek and gum. Reynolds also has introduced dissolving tobacco _ finely milled tobacco shaped into orbs, sticks and strips _ in test markets.

Swedish snuff more addicting?

The usual level for snuff or 'snus' is eight milligrams per gram but double that amount was found in one product produced by Swedish Match, the news program Kalla Fakta reported. The report alleges that Swedish Match added a substance known as E500 which raises "free" nicotine to increase craving. The tobacco company denies any wrongdoing. "There is no secret substance in snus," spokesman Henrik Brehmer said regarding the use of E500. "We use it to stabilize the pH value in snus and have done so for 200 years." Brehmer rejected an assertion by Harvard School of Public Health Professor Greg Connolly Swedish Match is deliberately using the substance to addict consumers. "In a study in 2008 he concluded that we are manipulating the pH value, something that we consider hugely speculative," Brehmer said.

A once solid and athletic guy, now in his late 40s, he'd lost more than 50 pounds and his skin was the color of fireplace ash. A long, thin scar ran from his jaw all the way down his neck and disappeared under his shirt. "I've got cancer," he said after re-introducing himself. He explained that he'd been through extensive surgery and treatments but that he was optimistic he'd pull through. Doctors told him smoking was the likely cause of his throat cancer. This week marks the two-year anniversary of the law that made it illegal to smoke in Minnesota's bars, restaurants and other public places. Olmsted County can take pride in the fact that it helped lead the way in the push to clear the air in the state's workplaces, eating establishments and watering holes. In November of 2001, Olmsted County Board members passed a comprehensive anti-smoking ordinance. The countywide ban was a courageous, landmark decision that helped create momentum for the statewide legislation. I don't know about you, but whenever I visit a state like Wyoming, where smoking is prohibited only in underground mines and other places where you could blow yourself up if you lit a match, I'm thankful for Minnesota's ban. New tobacco product prompts concerns… No it’s not Marlboro…

But the country’s latest mark on the U.S. is not so benign: an oral tobacco product known as snus. The moist tobacco, which comes in a tea-bag-style pouch that goes under the upper lip, has public health experts divided. Some say snus (rhymes with loose) could save scores of lives if smokers switch to it, because the product contains far fewer carcinogens than do cigarettes, chew and snuff. Critics, though, say snus will only increase the number of people addicted to nicotine and poses a serious threat to anti-tobacco efforts. They say it could create a new crop of young nicotine addicts, while smokers could just as easily supplement their habit rather than substitute one for the other.Some Americans have been using snus by ordering it online or buying it in U.S. test markets, but R.J. The top tobacco company across America will launch new snus variety under its best-selling brand, Marlboro, according to the chief executive of Altria Group based in Henrico County, VA. In addition, Philip Morris USA will attempt to expand its presence on the market of menthol cigarettes, by introducing a menthol extension of the mainstream discount brand L&M, Szymanczyk said during the Barclays Consumer Conference. And in the nearest future, Altria plans to launch another smokeless tobacco product, using its subsidiary, in order to capture more customers who prefer oral tobacco items. Altria CEO declared that they are currently working on the introduction of new variety of Copenhagen moist snuff with wintergreen flavor. According to industry analysts, the introduction of additional variety will contribute to approximately 9 percent increase of Copenhagen market share, from current 23% to almost 32%. J. Reynolds, whose brands include Camel, at the beginning of 2008. Now the Camel logo is back prominently in major glossies, including Rolling Stone, Sports Illustrated and Maxim — but not to advertise cigarettes. R. J. Reynolds is advertising Camel Snus, a tobacco packet that wedges in the upper lip and, unlike chewing tobacco, is promoted as “spitless” because low salt content spares users the unpleasantness of public expectoration. Although snus is popular in Sweden, this is the first time it has been marketed in the United States by a major American tobacco company. The campaign, by Quaker City Mercantile in Philadelphia, pitches Camel Snus (pronounced snoose) as a way around smoking bans. Oral use of Swedish moist snuff (snus) and risk for cancer of the mouth, lung, and pancreas in male construction workers: a retrospective cohort study

Scandinavian moist snuff (snus) has been proposed as a less harmful alternative to smoking, but precise data on the independent associations of snus use with site-specific cancers are sparse. We aimed to assess the risks for cancer of the oral cavity, lung, and pancreas. Methods: Detailed information about tobacco smoking and snus use was obtained from 279 897 male Swedish construction workers in 1978—92. Complete follow-up until end of 2004 was accomplished through links with population and health registers. To distinguish possible effects of snus from those of smoking, we focused on 125 576 workers who were reported to be never-smokers at entry. Adjusted relative risks were derived from Cox proportional hazards regression models. Findings: 60 cases of oral, 154 of lung, and 83 of pancreatic cancer were recorded in never-smokers. Snus use was independently associated with increased risk of pancreatic cancer (relative risk for ever-users of snus 2·0; 95% CI 1·2—3·3, compared with never-users of any tobacco), but was unrelated to incidence of oral (0·8, 95% CI 0·4—1·7) and lung cancer (0·8, 0·5—1·3). Interpretation: Use of Swedish snus should be added to the list of tentative risk factors for pancreatic cancer. We were unable to confirm any excess of oral or lung cancer in snus users. R. J. Reynolds introducing ‘dissolvable’ tobacco

Like Camel Snus, tea-bags filled with mint-flavored tobacco. R.J. Reynolds says Snus have become so popular, they're taking the next step -- totally dissolvable tobacco The company says it will solve all kinds of problems for traditional smokers. "They don't have second hand smoke. They don't have a litter problem. The product actually dissolves in your mouth as opposed to having to spit or extract something like a patch from your mouth like other smokeless products," says Tommy Payne of R. J. Reynolds. The company will soon test three new products: Camel Sticks that dissolve when you suck them, Minty Tobacco Strips that look like breath strips, and Orbs, flavored dissolvable tablets that some say look and taste like candy. Critics say R. J. Reynolds is doing what it did with Joe Camel -- marketing not to adult smokers, but smoker wanna-bes. "Really what you're doing with kids actually, it's kind of like a gateway drug. You're getting them addicted to nicotine, which then leads them to possibly wanting to do other things," says Dan Smith of the American Cancer Society.
