HPV infection may be linked to poor head and neck cancer survival rates in African Americans

Furthermore, African Americans had far less HPV infection than whites, which led to worse survival. "There is currently no consensus on why blacks fare worse with squamous cell carcinoma of the head and neck than whites, but this is the first clue that it may be biologic rather than related to issues of access, insurance or provider attitudes," said senior author Kevin Cullen, M.D., director of the University of Maryland Marlene and Stewart Greenebaum Cancer Center and professor of medicine at the University of Maryland School of Medicine. Cullen's research showed that median overall survival was more than threefold higher for whites (70.6 months) than for African Americans (20.9 months) who were treated with chemotherapy and radiation. When the researchers examined patients by HPV status, they found that HPV-negative patients had a median survival of 26.6 months, while the survival rate for HPV-positive patients could not be calculated because most were still alive. Overall, 4 percent of African American patients and 34 percent of white patients were HPV positive. Cullen said the survival difference was entirely due to HPV status, as survival rates were similar among HPV-negative patients. Led by researcher Per Jemth of Uppsala University in Sweden, the collaborators from four institutions detail in the Journal of Biological Chemistry how an offensive protein generated by the sexually transmitted virus handicaps a defensive protein made by the human body. Co-author Neil Ferguson, a biophysicist at University College Dublin, says: "It has proved difficult to stem the proliferation of many viruses using conventional drug discovery. Inhibitors of protein-protein interactions, as in HPV's case, are potentially potent ways to perturb viral infections." There are almost 200 strains of HPV, dozens of which are transmitted through genital contact, and about half of sexually active people have had one or more infections. The immune system eliminates the virus within two years in about 90 percent of cases, according to the Centers for Disease Control and Prevention in Atlanta, but it lingers for many years in a minority of cases. Some strains result in no visible symptoms, others cause genital warts and still others cause cancer. HPV: To test or not to test

Usually, we are happy to report that the tissue is healthy and no further treatment is necessary. Ethically, our role in assessment is both an understood practice as well as a legal one each time we interact with our patients. Within your practice, what is the expected role in assessing and relaying advice/recommendations to our patients? Do we rely strictly on scientific evidence with evidence-based protocol? Do we trust the developers/promoters of dental products to provide information to us? Do we relay personal opinion from colleagues, or do we reject opinion-based information? But this is no ordinary workspace, and some day it may merit a plaque. Second primary cancers after an index head & neck cancer: subsite-specific trends in the era of hpv–associated oropharyngeal cancer

Hayes and Ian Ganly Abstract Purpose Patients with head and neck squamous cell carcinoma (HNSCC) are at elevated risk of second primary malignancies (SPM), most commonly of the head and neck (HN), lung, and esophagus. Our objectives were to identify HNSCC subsite-specific differences in SPM risk and distribution and to describe trends in risk over 3 decades, before and during the era of human papillomavirus (HPV) –associated oropharyngeal SCC. Methods Population-based cohort study of 75,087 patients with HNSCC in the Surveillance, Epidemiology, and End Results (SEER) program. SPM risk was quantified by using standardized incidence ratios (SIRs), excess absolute risk (EAR) per 10,000 person-years at risk (PYR), and number needed to observe. Trends in SPM risk were analyzed by using joinpoint log-linear regression. Results In patients with HNSCC, the SIR of second primary solid tumor was 2.2 (95% CI, 2.1 to 2.2), and the EAR was 167.7 cancers per 10,000 PYR. The risk of SPM was highest for hypopharyngeal SCC (SIR, 3.5; EAR, 307.1 per 10,000 PYR) and lowest for laryngeal SCC (SIR, 1.9; EAR, 147.8 per 10,000 PYR). The most common SPM site for patients with oral cavity and oropharynx SCC was HN; for patients with laryngeal and hypopharyngeal cancer, it was the lung. Most oral cancers are caused by smoking or drinking but Darren's cancer was caused by the Human Papillomavirus (HPV) which is sexually transmitted. Darren had caught it through having oral sex. And he's not alone. New research shows that there has been a dramatic increase in the number of HPV-related oral cancers amongst young people. Actress Jaime Winstone sets out to discover why the statistics are rising and whether anything can be done to stop this trend. Having met Darren, Jaime wants to know more. Douglas says his doctors told him he has an 80% survival rate if it hasn’t spread to his lymph nodes. While tobacco was the prime cause of oral cancer in the past, recent studies have attributed the steady increase of the disease to the human papillomavirus (HPV). HPV are common viruses that cause warts. New Discovery May Offer Cure for Human Papillomavirus (HPV)

Detection of human papilloma virus (HPV) in oral mucosa of women with cervical lesions and their relation to oral sex practices

The purpose of this investigation was to study the prevalence of HPV in oral cavity of women with oral sex practices and cervical lesions. Methods: Forty six (46) non-smokers and non-alcoholic patients attended the "Clinica de Displasias"of "Ciudad Juarez"were sampled. This population had a CIN (cervical intraepithelial neoplasia) diagnosis sometime between the previous six months. On previous consent they filled out a questionnaire related to their oral sex practices. Afterwards one swab from cheeks and another from palate/gum were taken; PCR (polymerase chain reaction) was used to determine generic HPV, HPV16 and HPV18. Results: Seventy two percent (72%) of the patients stated to have oral sex practices regularly which all of them were positive to HPV either in oral mucus, palate/gum or both. The total of the given results showed that 35% had HPV16; among those distributed in 26% with regular oral sex practices and 9% stated as never practiced oral sex. An association was found between oral HPV16 positivity and progression to CIN advanced lesions. On the other hand HPV18 was not detected. The frequency of HPV16 was higher in buccal mucosa (23%) versus palate/gum (16%). Conclusions: This study suggests that buccal HPV16 infection is associated with CIN progression.
