Human papillomavirus knowledge and vaccine acceptability among a national sample of heterosexual men

We aimed to characterise heterosexual men's willingness to get HPV vaccine and identify correlates of vaccine acceptability. Methods Participants were from a national sample of heterosexual men (n=297) aged 18–59 y from the United States who were interviewed in January 2009. We analysed data using multivariate logistic regression. Results Most men had not heard of HPV prior to the study or had low HPV knowledge (81%; 239/296). Most men had heard of HPV vaccine prior to the study (63%; 186/296) and 37% (109/296) were willing to get HPV vaccine. Men were more willing to get vaccinated if they reported higher perceived likelihood of getting HPV-related disease (OR 1.80, 95% CI 1.02 to 3.17), perceived HPV vaccine effectiveness (OR 1.86, 95% CI 1.22 to 2.83) or anticipated regret if they did not get vaccinated and an HPV infection later developed (OR 2.01, 95% CI 1.40 to 2.89). Acceptability was also higher among men who thought (OR 9.02, 95% CI 3.45 to 23.60) or who were unsure (OR 2.67, 95% CI 1.30 to 5.47) if their doctor would recommend they get HPV vaccine if licenced for males. Charity backs HPV vaccine for boys

The British Dental Health Foundation said HPV is now a leading cause of cancer for men and that more must be done to prevent it, starting with extending the HPV vaccine to boys. The charity's chief executive Nigel Carter said: ‘We strongly recommend that the government take another look at introducing the vaccination for young males. 'Much of the 42% rise in incidence of mouth cancer over the last 10 years is attributed to HPV and whilst vaccination of adolescent females will help, in order to be truly effective we will need to consider vaccination of males as well.' The debate had reignited following an Australian study that found almost two thirds of all throat and tonsil cancer cases are caused by the HPV virus. Research now suggests HPV, which is transmitted by oral sex, could soon rival smoking and drinking as a main cause of mouth cancer, the charity said. The national HPV vaccine programme has vaccinated 1.5 million teenage girls since September 2008. A similar programme for boys was turned down because the cost exceeded the upper threshold of NICE's cost-effectiveness benchmark. Mr Carter said: ‘The impact of the disease is huge and treatment of advanced cases extremely expensive so it is likely that a cost effective case for the vaccination could be put forwards.'

HPV vaccine fails to get nod for routine use in boys

Instead, the Advisory Committee on Immunization Practices (ACIP) issued a "permissive" recommendation stating that physicians have the option of vaccinating boys with the HPV vaccine Gardasil(Merck) for genital-wart protection. ACIP noted that boys should receive the vaccine "before exposure to HPV through sexual contact." In 2006, the US Food and Drug Administration (FDA) approved Gardasil for use in girls and young women to prevent cervical and other cancers as well as precancers caused by HPV types 16 and 18. Last week, the FDA also approved Gardasil for use in males aged 9 through 26 to prevent genital warts. In making its recommendation today, ACIP appeared to be swayed in part by published research questioning the cost-effectiveness of routine vaccination of boys with Gardasil. The committee's decision has far-reaching economic effects because third-party payers typically pay for vaccinations that are recommended for routine use, but not those coming with a permissive recommendation. It protects against the human papillomavirus, or HPV, which causes a variety of cancers and genital warts. But the committee did recommend including Gardasil for eligible boys aged 9 to 18 in the Vaccines For Children program, a government-funded system that provides vaccines to children eligible for the state-federal Medicaid health insurance plan and other uninsured children. Last month, a U.S. Food and Drug Administration panel recommended expanding use of the Gardasil vaccine to males aged 9 to 26 to protect them from genital warts, but the cost effectiveness of such a program was unclear at the time. But researchers from the Harvard School of Public Health conducted such an analysis comparing a girls-only vaccination program with a co-ed vaccination program. Currently, Gardasil is approved for girls aged 9 and over to protect them from cervical cancer. "This study found that while vaccine coverage and efficacy are high in girls, including boys in an HPV vaccination program generally exceeds what the U.S. typically considers good value for money," said lead researcher Jane Kim, an assistant professor of health decision science. The report is published in the Oct. 9 online edition of the British Medical Journal. Vaccination was considered a good value if cost-effectiveness ratios ranged from $50,000 to $100,000 per quality-adjusted life year, meaning the cost of the vaccine vs. the number of added years someone would gain by getting the vaccine. New broad-spectrum vaccine to prevent cervical cancer induces strong responses in animals

Source: Journal of the National Cancer Institute Author: Staff Wednesday, May 27, 2009   Mice and rabbits immunized with a multimeric-L2 protein vaccine had robust antibody responses and were protected from infection when exposed to human papillomavirus (HPV) type 16 four months after vaccination, according to a new study published in the May 26 online issue of the Journal of the National Cancer Institute. Current HPV L1-based vaccines are almost 100% protective against infection by the two HPV types that are responsible for 70% of all cervical cancer cases world wide. However, the existing vaccines provide limited protection against the other HPV types that cause cancer. With that limitation in mind, Richard Roden, Ph.D., of the Johns Hopkins University in Baltimore, and colleagues have been working on an alternate vaccine that is based on the HPV minor capsid protein L2, which is highly conserved between HPV types. Previous experiments showed that the L2 protein induced only a weak antibody response in animals. In the current study, Roden and colleagues linked together a short segment of the L2 protein from several HPV types to generate a single multimeric L2 fusion protein. They tested the ability of this multimeric-L2 protein to induce antibody responses in animals and its ability to protect them from subsequent infection with HPV type 16. Trial shows cancer vaccine success

Women aged from 24 to 45 who have not already been infected with the human papilloma virus (HPV) can be protected by the HPV vaccine, according to an article published in the medical journal The Lancet.   HPV is a sexually transmitted disease which is responsible for most forms of cervical cancer. Trials were conducted with over 3,000 women in the relevant age group who had not been infected with HPV. Half were given a placebo and half were given a vaccine which protects against four strains of HPV. The researchers found that four cases of infection occurred in the vaccine group compared to 41 in the placebo group – meaning the vaccine was 91 per cent effective against all four virus strains. Looking at HPV 16 and 18 only, which are responsible for 70 per cent of cases of cervical cancer, four cases occurred in the vaccine group compared with 23 in the placebo group, meaning the vaccine was 83 per cent effective. When trials were conducted with women who had already been infected with HPV, the vaccine was only 30 per cent effective. Why was the vaccine against the cancer-causing, sexually transmitted HPV virus not approved for men at the same time it was for women? Is there any reason to think that men and women would react to it differently? Interesting question. First, a little background. Gardasil is a genetically engineered vaccine to immunize girls and women ages 9 to 26 against four types of human papillomavirus (HPV). HPV is the name given to a family of viruses, many of which can be transmitted from one partner to another through sexual activity. HPV may represent the most frequent sexually transmitted infection in the United States. It is estimated that over 6 million people become infected by a sexually related form of HPV every year. Some estimates suggest that over half of all sexually active males and females become infected at one point or another in their lifetimes. Certain types of HPV can sometimes evade the body's immune system and, when they do, establish a state of persistent infection. That in turn may cause certain cancers. Indeed, this is now recognized as the major cause of cervical cancer and related conditions, and vaccines that immunize people against HPV could make a dramatic impact against these diseases.
