Miriam Rosin, a BC Cancer Agency scientist, said the actor’s candid revelation that his throat cancer was caused by human papillomavirus (HPV), which he picked up from performing cunnilingus, is raising awareness of a growing problem around the world, and in B.C. “It’s created a lot of noise. I think it’s important to talk about this disease … but not in a headline-grabbing way, which may damage the cause by labelling it as a sexually transmitted disease,” said Rosin, who is also a Simon Fraser University professor. Regardless, the public is finally getting the message that HPV, the most common sexually transmitted virus in the world – and the one that causes virtually all cases of cervical cancer – is accounting for the surge in throat cancers located at the back of the throat. In B.C., if trends continue, HPV-caused throat cancers are expected to overtake cervical cancers in incidence. About 150 cases of cervical cancers are reported annually in this province. HPV vaccine still fights for acceptance, despite benefits

He could be right. Although smoking and alcohol use have long been regarded as the key risk factors, new research indicates that HPV, a sexually transmitted virus, is now the leading cause of mouth and throat cancers in the United States. But there’s an important take-away message to this story: Some cancers caused by HPV can be prevented easily, with a simple series of three vaccinations. Since 2000, scientists have known that certain strains of HPV are responsible for nearly all cervical cancer in women. But newer studies indicate HPV can cause other types of cancer as well. Recent findings have also linked HPV to oral, head/neck, anal, vaginal, vulvar and penile cancers, and even some cases of lung cancer. About half of all Americans will become infected with HPV at least once during their lifetime. The most common visible symptom of an HPV infection is genital warts, although the majority of HPV infections do not display symptoms. That’s why the Centers for Disease Control and Prevention recommends that boys and girls alike be vaccinated against HPV. Ideally, they should be vaccinated between the ages of 11 and 12. Oral cancer sneaks up

By asserting last week that his cancer was caused by a virus transmitted during oral sex, Mr. Douglas pushed the disease onto the front pages and made millions of Americans worry about it for the first time. In this case, it was a subset of Americans who normally worry more about being killed by cholesterol than by an S.T.D. The typical victim is a middle-aged, middle-class, married heterosexual white man who has had about six oral sex partners in his lifetime. The virus, human papillomavirus Type 16, also causes cervical cancer. So is there any early oral screening that a man can have — an equivalent to the Pap smear, which has nearly eliminated cervical cancer as a death threat in this country? The answer, according to cancer experts and a recent opinion from the United States Preventive Services Task Force, is no. And for surprising reasons. The Pap test — invented in 1928 by Dr. George N. Papanicolaou — involves scraping a few cells from the cervix and checking them under a microscope for precancerous changes. Public lacks awareness of head and neck cancer

There were nearly 40,000 new cases of oral, head and neck cancer diagnosed last year in the USA, of which 85 per cent could be linked to tobacco use and heavy alcohol consumption. Yet, according to a survey from the Medical University of South Carolina, the public remains largely unaware of the risks. In the study, over 1,000 members of the public were telephoned and 62 per cent said they were not knowledgeable about OHNC. Among smokers, the lack of awareness ran at 58 per cent. Under half of non-smokers and smokers knew that smoking was a risk factor. Hoarseness was correctly identified as a symptom by only one per cent of smokers and two per cent overall. But 17 per cent of the sample incorrectly named headache as a symptom. People can spot the signs and symptoms of OHNC themselves, but 94 per cent had not been told to look for problems like mouth sores that do not heal. And only 26 per cent had been check for this by a doctor. Clearly there is some way to go in raising awareness among the public of OHNC.

Study examines role of DNA, HPV in oral cancer survival

However, HPV-positive OPSCC is highly curable, and patients with HPV have better survival compared with HPV-negative patients, whose cancers are usually associated with alcohol and tobacco use. To better understand the molecular mechanisms underlying these differences, Jochen Hess, PhD, and colleagues at University Hospital Heidelberg monitored changes in DNA modifications in HPV-positive and HPV-negative OPSCCs (Journal of Clinical Investigation, May 1, 2013). They applied an array-based approach to monitor global changes in CpG island hypermethylation between HPV-negative and HPV-positive OPSCCs, and identified a specific pattern of differentially methylated regions that critically depends on the presence of viral transcripts. This DNA modification pattern was significantly correlated with improved survival in three separate groups of OPSCC patients, the researchers noted. "Our study highlights specific alterations in global gene promoter methylation in HPV-driven OPSCCs and identifies a signature that predicts the clinical outcome in OPSCCs," they concluded.

What the ‘rinse-and-spit’ oral cancer test could mean for dental professionals and their patients

The test, developed at the University of Miami Miller School of Medicine and exclusively licensed to Vigilant Biosciences, Inc., will be available as a low-cost, point-of-care screening test for the early detection of oral cancer, and is envisioned as a kit with a special oral rinse and test strip. The test strip detects proteins that are markers of oral cancer and are captured by the rinse. The markers may be present before a lesion is easily visible. Early detection tests are critical because the majority of patients present in late stage when cure rates reach only 40%. Study shows Vigilant Biosciences’ simple “rinse and spit” test effective in detecting oral cancer

Decoding the oral leukoplakia/oral cancer link

Do all oral leukoplakias lead to oral squamous cell carcinoma (OSCC)? Is it possible to detect premalignant oral leukoplakia? These are some of the questions a recent literature review in Oral Diseases attempted to answer (January 11, 2013). A team of researchers from Italy, Spain, the U.K., and the U.S. did a literary search of Medline/PubMed, Embase, and Best Evidence from January 1966 to June 2012. Search terms included leukoplakia, oral leukoplakia, preneoplastic oral, precancerous oral, oral precancerous, oral dysplasia, oral mucosal lesion, proliferative verrucous leukoplakia, multifocal leukoplakias, tobacco, and alcohol. The searches were designed to help the study authors address four key questions: 1.Do tobacco and alcohol cause oral leukoplakias? 2.What percentage of oral leukoplakias evolve into OSCC? 3.Can practitioners distinguish between premalignant and innocent oral leukoplakias? 4.Is proliferative verrucous leukoplakia (PVL) a specific entity or just a form of multifocal leukoplakia? For the purposes of this study, the term oral leukoplakia was used to recognize "predominantly white plaques of questionable risk, having excluded (other) known diseases or disorders that carry no increased risk of cancer."
