Davis Parents who face the dilemma of whether to protect their young daughters with a vaccine aimed at a sexually transmitted infection that causes cervical cancer now face a new question: Should they do the same for their sons? As evidence mounts of a rising number of other cancers linked to the human papillomavirus, or HPV, a debate has intensified over whether to give the vaccine to males. Advocates say vaccinating boys and men can prevent them from passing on the virus to their sexual partners. Critics still question the long-term safety and effectiveness of Merck & Co.’s Gardasil, despite studies indicating that its risks and lasting power are within the range of other vaccines. But a newer wrinkle in the debate is the discovery in recent years that oral HPV infections — most likely acquired from oral sex with multiple partners—significantly increase the risk of head and neck cancers. The rate of oral cancers is rising so steadily, especially in men, that, if the trend continues, there may be more oral cancers in the United States caused by HPV in 10 years than by tobacco or alcohol, a major study concluded last year. “We should be investing our care and dollars in preventing HPV infection instead of treating the cancers,” said Dr. Thom Loree of Roswell Park Cancer Institute. New diagnostic advance seen for head, throat cancer

Source: www.sciencedaily.com Author: staff Pharmacy researchers at Oregon State University today announced the discovery of a genetic regulator that is expressed at higher levels in the most aggressive types of head and neck cancers, in work that may help to identify them earlier or even offer a new therapy at some point in the future. This "transcriptional regulator" is called CTIP2, and in recent research has been demonstrated to be a master regulator that has important roles in many biological functions, ranging from the proper development of enamel on teeth to skin formation and the possible treatment of eczema or psoriasis. In the newest study, published April 28 in PLoS One, scientists found for the first time that levels of CTIP2 were more than five times higher in the "poorly differentiated" tumor cells that caused the most deadly types of squamous cell carcinomas in the larynx, throat, tongue and other parts of the head. There was a high correlation between greater CTIP2 expression and the aggressive nature of the cancer. Head and neck squamous cell cancers are the sixth most common cancers in the world, the researchers said in their study, and a significant cause of mortality. In 2008, cancers of the oral cavity and pharynx alone accounted for 35,310 new cases in the United States and 7,590 deaths. A visit to the dentist is crucial and could mean the difference between life and death; someone in the United States dies of oral cancer nearly every hour, according to the Oral Cancer Foundation. Early detection is crucial in the fight against oral cancer, an aggressive, deadly disease that hasn’t seen the same improvements in survival rates as other cancers, said Parish Sedghizadeh of the USC School of Dentistry. “Oral cancer has one of the highest mortality rates among cancers,” said Sedghizadeh, assistant professor of clinical dentistry at USC. “It’s usually not noticed until the later stages, when a recovery is less likely. People have heard of oral cancer, but they don’t know what it looks like.” The disease rarely causes pain or other noticeable symptoms until it reaches a very advanced stage, he said. Diet, nutrition, and cancer — don’t trust any single study

Source: American Association for Cancer Research (AACR) 100th Annual Meeting Author: Zosia Chustecka Numerous studies on diet and cancer were presented here at the American Association for Cancer Research (AACR) 100th Annual Meeting, but several of the findings that were highlighted in AACR press releases — and thus are likely to be picked up by the lay media — run counter to the accumulated body of evidence, and some of the comments based on these studies are untrue or premature. So said Walter Willet, MD, DrPH, from the department of nutrition at Harvard School of Public Health, in Boston, Massachusetts, in an exclusive interview with Medscape Oncology. "No conclusions should be made on the basis of a single study," he said. Dr. Willett presented an overview entitled "Diet, Nutrition, and Cancer: The Search for Truth," in which he reviewed many of the associations that have been suggested by epidemiologic studies. These include consumption of red meat, meat cooked at a high temperature, a high-fat diet, and alcohol all increasing the risk, and fruit and vegetables decreasing the risk. However, much of the evidence for these links is rather weak, he said; the most robust evidence supports a link between obesity and an increased risk for cancer. Abstract Design Two hospital-based case–control studies were conducted in central and eastern Europe and Latin America. Case/control selection Cases and controls were recruited in Moscow (Russia), Bucharest (Romania) and Lodz and Warsaw (Poland) from 1998 to 2002, and from 1998 to 2003 in Buenos Aires (Argentina), La Habana (Cuba) and the Brazilian towns Rio de Janeiro, São Paulo, Pelotas, Porto Alegre and Goiânia. Incident cases of squamous cell carcinoma of the head and neck (oral cavity, pharynx, larynx) and oesophagus, as well as age- (in quinquennia) and sex frequency-matched controls, were enrolled from 1998 to 2003. Control subjects included residents of the study areas for at least 1 year who were admitted to the same hospitals as the cases or in a comparable catchment area (controls in São Paulo were not recruited from oncology hospitals, and population-based controls were enrolled in Warsaw). Controls were hospitalised for prespecified conditions thought to be unrelated to tobacco use or alcohol consumption. Pretreatment health behaviors predict survival among patients with head and neck squamous cell carcinoma

Purpose: Our prior work has shown that the health behaviors of head and neck cancer patients are interrelated and are associated with quality of life; however, other than smoking, the relationship between health behaviors and survival is unclear. Patients and Methods: A prospective cohort study was conducted to determine the relationship between five pretreatment health behaviors (smoking, alcohol, diet, physical activity, and sleep) and all-cause survival among 504 head and neck cancer patients. Results: Smoking status was the strongest predictor of survival, with both current smokers (hazard ratio [HR] = 2.4; 95% CI, 1.3 to 4.4) and former smokers (HR = 2.0; 95% CI, 1.2 to 3.5) showing significant associations with poor survival. Problem drinking was associated with survival in the univariate analysis (HR = 1.4; 95% CI, 1.0 to 2.0) but lost significance when controlling for other factors. Low fruit intake was negatively associated with survival in the univariate analysis only (HR = 1.6; 95% CI, 1.1 to 2.1), whereas vegetable intake was not significant in either univariate or multivariate analyses. Although physical activity was associated with survival in the univariate analysis (HR = 0.95; 95% CI, 0.93 to 0.97), it was not significant in the multivariate model. Drink a day increases cancer risk

Consuming just one drink a day causes an extra 7,000 cancer cases - mostly breast cancer - in UK women each year, Cancer Research UK scientists say. The risk goes up the more you drink, whether spirits, wine or beer, the data on over a million women suggests. Overall, alcohol is to blame for about 13% of breast, liver, rectum, mouth and throat cancers, the researchers say. They estimate that about 5,000 cases of breast cancer in the UK - 11% of the 45,000 cases diagnosed each year - can be attributed to women's consumption of alcohol. The study looked specifically at women who consumed low to moderate levels of alcohol - defined as three drinks a day or fewer. Over the seven years of the study, published in the Journal of the National Cancer Institute, a quarter of the 1.3 million women reported drinking no alcohol. Of those who did drink, virtually all consumed fewer than 21 drinks per week, and an average of 10g of alcohol per day, which is equivalent to just over one unit of alcohol found in half a pint of lager, a 125ml glass of wine or a single measure of spirits. More evidence links alcohol, cancer in women

British researchers surveyed middle-aged women at breast cancer screening clinics about their drinking habits, and tracked their health for seven years. A quarter of the women reported no alcohol use. Nearly all the rest reported fewer than three drinks a day; the average was one drink a day. Researchers compared the lightest drinkers - two or fewer drinks a week - with people who drank more. Each extra drink per day increased the risk of breast, rectal and liver cancer, University of Oxford researchers reported Tuesday in the Journal of the National Cancer Institute. The type of alcohol - wine, beer or liquor - didn't matter. That supports earlier research, but the new wrinkle: Alcohol consumption was linked to esophageal and oral cancers only when smokers drank. Also, moderate drinkers actually had a lower risk of thyroid cancer, non-Hodgkin's lymphoma and renal cell cancer. For an individual woman, the overall alcohol risk is small. Tempest in a bottle of mouthwash

Rinsing with mouthwash doesn’t just provide a scrubbed, minty feeling; it’s good for our health, we’re told, curbing plaque and gingivitis (not to mention bad breath). Some brands even carry the Canadian Dental Association’s official seal. But this so-called healthy habit could be doing more harm than good. Australian researchers recently concluded that mouthwashes containing alcohol may contribute to oral cancer. Tobacco use is the biggest risk factor for oral cancer, according to the Canadian Cancer Society. Combined with excessive drinking, it’s even more dangerous—a heavy smoker and drinker is up to 30 times more likely to develop it. Even so, “there’s a small group of patients who don’t seem to have any risk factors,” says Michael John McCullough, an associate professor at the Melbourne Dental School and one of the experts behind the report. “I noticed some were saying they’d used alcohol-containing mouthwashes over a long period of time.” In the article, published in the Australian Dental Journal in December, McCullough and co-author Camile Farah conclude there is now “sufficient evidence” to suggest these mouthwashes are a contributing factor. Row erupts over oral cancer paper authorship

The research paper's co-authors say Newell Johnson, whose Griffith University laboratory was funded by pharmaceutical firm Pfizer, Listerine's recent owner, decided not to put his name to the research paper, which made headlines across the world with its finding that alcohol-based mouthwashes were implicated in oral cancer. Professor Johnson says he was never an author. The claim about his involvement and withdrawal, made by Australian co-authors Michael McCullough of the University of Melbourne and Camile Farah of the University of Queensland, adds a new dimension to the controversy ignited by the paper, published in the Australian Dental Journal last December. In January UQ's head of dentistry Laurence Walsh came to the defence of mouthwashes, arguing they might prevent oral cancer, but later conceded that Listerine's present owner, Johnson & Johnson, had sponsored some of his workshops.
