This poor outcome is due to several factors, including delayed diagnosis. Therefore, improved early detection and effective prevention strategies are critical components for management of this malignancy. The etiology of classical HNSCC has been attributed to chronic exposure to tobacco and alcohol.2 In addition, there is now sufficient evidence to support the contention that high-risk forms of the human papillomavirus (HPV) are a major causative factor for HNSCC of the tonsil, base of tongue, and oropharynx.3-8 The increasing importance of HPV in oropharyngeal HNSCC has raised considerable concern and uncertainty among healthcare professionals and patients. For example, I am often asked to describe the clinical features of HPV-associated premalignant lesions. Unfortunately, to my knowledge, the clinical spectrum of HPV-associated premalignant disease has not been adequately described. Furthermore, because this subset of HNSCC often develops in hard to- examine locations, such as tonsillar crypts, it can be exceedingly difficult to even identify the carcinomas. High-risk human papillomavirus in esophageal squamous cell carcinoma

Source: cebp.aacrjournals.org Authors: Annika Antonsson et al Background: Although most cases of esophageal squamous cell carcinoma (ESCC) in western populations have been attributed to high levels of exposure to tobacco and alcohol, infectious agents have been postulated as possible causes, particularly human papillomavirus (HPV). Methods: To explore this issue, we analyzed HPV DNA prevalence and HPV types together with lifestyle factors, in relation to tumor stage and survival in a low-incidence population. Archived tumor samples from a nationwide cohort of 222 ESCC patients were tested for the presence of HPV DNA by PCR; positive samples were sequenced to determine HPV type, and p16INK4a status was assessed by immunohistochemistry. Results: Of 222 ESCC patients, 8 tested HPV positive (prevalence, 3.6%; 95% confidence interval, 1.1-6.1%), of which 6 were HPV-16 positive and 2 were HPV-35 positive. Four of the eight HPV-positive tumors overexpressed p16INK4a. None of 55 normal esophageal tissue samples from healthy participants had any detectable HPV. Although the numbers were low, it seemed that patients with HPV-positive ESCC tumors were younger than those with HPV-negative tumors (mean age, 60.8 versus 65.3 years, P = 0.18) and had higher body mass index (BMI) throughout life (mean current BMI of 25.1 for HPV positive, 22.2 for HPV negative, P = 0.08; mean BMI at 20 years of 25.8 for HPV positive, 22.1 for HPV negative, P = 0.003). Is drinking coffee as good as it is made out to be?

Nine existing studies were looked at and analyzed. Researchers determined how much of coffee was consumed by more than 5,000 cancer patients and 9,000 healthy people. In the latest study conducted, researchers have found that the incidence of head and neck cancers appeared to decrease when four or more cups of coffee were drunk every day. The risk in such cases decreased by 39 percent in respect of oral cavity and pharynx cancers. However, it did not appear to have any effect on laryngeal cancer. By and large, the results seemed really positive, but Mia Hashibe, lead researcher and an assistant professor in the department of family and preventive medicine at the University of Utah, cautioned against drinking lots of coffee. Looking at the results from a logical point of view, Hashibe pointed out that the main risk factors for oral cancers were smoking and drinking alcohol, and the best way to prevent such cancers would be to stay away from smoking and drinking alcohol. Professor: Oral cancer deadlier than most

Dr. Karen Moak Crews, a 1975 graduate of McComb High School, is oral oncology and bio behavioral medicine director at UMC as well as a professor. Speaking to the McComb Rotary Club Wednesday, she warned against tobacco use of any type, including smokeless tobacco. Tobacco is the No. 1 cause of diseases and death in America, she said. Her main topic was oral cancer, which she said doesn’t get the publicity of some of the other cancers but is deadlier than most. The five-year survival rate for cancers in the mouth is 50 percent, she said, adding a big problem is late diagnosis. She urged regular screening by dentists for the disease. Risk factors, she said, include age, genetic mutation, nutrition, a sexually spread virus, alcohol and tobacco use. Citing statistics that alcohol in moderation — two ounces daily for a male and one ounce for a female — is considered safe by medical professionals, Crews said “there is no safe level for tobacco.”

Mouth cancer patients ‘ignore’ symptoms

The research, which interviewed relatively young mouth cancer patients in Scotland, found that most had heard of oral cancer but they didn't think their symptoms were indicative of the life-threatening illness. Furthermore, 40% of participants decided to self manage their symptoms and sought over-the-counter treatments which were suggested by a pharmacist. Chief executive of the British Dental Health Foundation, Dr Nigel Carter says the study further confirms gaps in understanding and awareness of oral cancer. Dr Carter explains: 'Public awareness of oral cancer and the associated risk factors appears to be too low here in the UK. 'An awareness of the risk factors and symptom recognition by the public is a critical issue in determining survival rates, as early detection greatly improves the chances of survival.' Almost 9% of patients who were interviewed had a prior knowledge of oral cancer and the causes such as tobacco and alcohol. However, this prior knowledge was neither instrumental for them to suspect they may have mouth cancer nor did it prompt them to visit a healthcare professional in the first place. ADA unveils oral cancer evidence-based recommendations

Source: www.ada.org/news Author: Jennifer Garvin A panel convened by the ADA Council on Scientific Affairs explored the potential benefits and risks of screening for oral squamous cell carcinomas and the use of screening aids to detect malignant or potentially malignant oral lesions. The panel's findings are published as the cover story in the May edition of The Journal of the American Dental Association. "Evidence-Based Clinical Recommendations Regarding Screening for Oral Squamous Cell Carcinomas" were developed by a CSA expert panel convened in April 2009, and join similar recommendations on topical fluoride and sealants as the Association's only evidence-based recommendations. Though evidence-based dentistry (EBD) recommendations do not represent a standard of care, the CSA hopes practitioners will use the recommendations as a resource in their clinical decision-making process alongside a clinician’s judgment and experience in the context of a patient's individual needs. The panel worked with ADA Center for Evidence-Based Dentistry staff and assessed five systematic reviews and four clinical studies as a basis for developing the recommendations. They addressed whether or not screenings help reduce morbidity and mortality, and whether or not oral cancer detection devices aid in detecting potentially malignant or malignant lesions. Is green tea the way to better oral health?

The study, which examined the dental health of 940 men, found that those who drank green tea regularly had better oral health than those who consumed less. Male participants, aged 49–59, were examined on three indicators of gum disease: researchers found that for every cup of green tea consumed per day, a decrease in all three indicators occurred. Chief Executive of the Foundation, Dr Nigel Carter, welcomed this new research and said: “Any new study which brings dental healthcare into the spotlight is a step in the right direction. “Like any study of a relatively small number of people we can never be certain of the results – but because tea is relatively cheap and easily available we must take notice of studies of this type and we at the Foundation will continue to watch and report on this and similar stories.” The British Dental Health Foundation has previously reported on studies which show a connection between drinking green tea and decreasing the growth of cancer cells in the body. Poor awareness of head and neck cancer

Source: www.imn.ie Author: Kay Kinsella Eighty-three per cent of those surveyed on their knowledge of head and neck cancer (HNC) have admitted to knowing little or nothing about the disease. The study of 200 Irish people, published in the Irish Medical Journal, showed the majority (96 per cent) of those surveyed identified smoking as a major risk factor to developing HNC, but few (27 per cent) recognised excessive alcohol consumption as a risk. Less than 100 (50 per cent) would have concern about persisting hoarseness or a prolonged oral ulcer, despite them being common symptoms of HNC. HNC is the sixth most common cancer worldwide, with more than half a million new cases diagnosed every year and 200,000 deaths resulting from this cancer annually. Survival rates of the cancer, however, remain low with 50 per cent of those diagnosed with tongue cancer dying within five years. Early diagnosis of HNC could boost survival rates up to 70-80 per cent, however, it is reported that 60 per cent of patients diagnosed with HNC are at an advanced stage of the disease. The study reveals that 98 per cent of the public surveyed desired more information about the disease, however, there is little awareness being raised on the topic among the Irish public. Alcohol based mouthwash and oral cancer – too much confusion

Source: scienceblogs.com Author: Francis Mawanda If you are like me, you probably always and almost faithfully, include a bottle of mouthwash on your grocery list especially after watching and/or listening to the numerous commercials in the media which claim that you will not only get long lasting fresh breath, but also freedom from the germs that cause plaque and gingivitis. However, many proprietary mouthwashes including my favorite brand contain Alcohol (ethanol) which also gives them the characteristic burn we have to endure, albeit for a few seconds each day, but safe in the knowledge that the product is hard at work killing all the germs that give us bad breath and may cause plaque and gingivitis. But the question I continually ask myself is whether regular or long term use of these products is safe especially after reading the numerous research reports and newspaper articles suggesting a possible link between long term use of alcohol based mouthwashes and oral cancer. Several research studies have reported finding an association between long term mouthwash use and oral cancer (1, 2, 3). For example, in a study conducted by Wynder and colleagues (1), they found a significant association between mouthwash use and oral cancer. A bigger multi-site study by Guha and colleagues (3) comparing participants who reported having used mouthwash to those who reported never having used mouthwash found that individuals who reported using mouthwash more than twice a day were nearly six times more likely to develop oral squamous cell carcinoma compared [...]

More than 35,000 cases of oral cancer are diagnosed in the United States annually, and more than 45 percent of those will not be alive in five years. To put it another way, oral cancer kills one person every hour, 24 hours per day, seven days per week in the U.S. You can you avoid becoming a statistic for this deadly disease by abstaining from tobacco and alcohol use, since 75 percent of all oral cancers are diagnosed in people who use tobacco products. Those who use tobacco and alcohol have an even higher risk. However, there has been a 60-percent increase over the past decade in the number of oral cancer diagnoses in people under the age of 30 with none of the classic risk factors listed, due to a connection with the HPV virus. Everyone must be screened. While the rates of colon, cervical and prostate cancer have seen major declines in recent years due to early detection through professional screening, the oral cancer survival rate has not increased significantly in the last 50 years. However, if oral cancer is detected and treated in its early stages, the five-year survival rate climbs to 80-90 percent. Areas of highest risk for oral cancer are the side of the tongue, floor of the mouth and back of the throat, so regular dental visits must include a comprehensive examination of the soft tissues at least annually.
