More than 40 patients have been treated for head and neck ­cancer during lockdown. Others have undergone procedures for facial skin cancer and facial injuries. The Royal College of Surgeons Edinburgh says 87% of its members in cancer surgery had stopped operating altogether or significantly reduced the number of procedures due to the danger of patients contracting the virus. And experts fear deaths from cancer could rise by a fifth over the next year as a result of scores of treatments and consultations being cancelled. Now it is hoped the protocols ­followed by the Glasgow team can be adopted to allow more surgery to resume. Critical to the safety of the operations is that the patients are tested for coronavirus twice before the procedure – once 48 hours beforehand, and then again immediately before surgery begins. Patients are also asked to ­self-isolate for two weeks before surgery, the surgical team works in a separate building to the main hospital and some surgical techniques have been modified to reduce the risk of infection. Dr. Michael Moore, director of head and neck surgery at UC Davis and an HPV-related cancer expert, answers some tough questions about the trend and what can be done about it. Q: What is HPV and how is it related to head and neck cancers? A: There are about 150 different types of HPV, but HPV 16 is the one that most frequently causes cancers that affect the tissue in the oropharynx, which includes back of the throat, soft palate, tonsils and the back or base of the tongue. You can get non-cancerous lesions from other types of HPV that look like warts in the nose, mouth or throat, called papillomas. Some can develop in childhood just from exposure early in life. Some develop later in life and only occasionally turn into cancer. Q: How do you get HPV? A: HPV can spread from mother to her baby around the time of delivery. Should women older than 18 get the HPV vaccine?

Should adult women follow suit? Yes, says Lauri Markowitz, a Centers for Disease Control and Prevention medical epidemiologist who has worked with the advisory committee that makes national vaccination recommendations. “Women 18 to 26 should be vaccinated.” There’s good reason to follow that recommendation. According to the American Cancer Society, about 12,820 new cases of cervical cancer will be diagnosed in U.S. women this year and more than 4,000 will die of the disease. HPV is thought to be responsible for more than 90 percent of all cervical and anal cancers in men and women. The virus also causes vaginal, vulvar and throat cancers and genital warts. Although the majority of HPV infections do not cause cancer — most people with an infection never show any symptoms, and infections usually go away on their own — some strains are particularly dangerous. Gardasil 9, the newest HPV vaccine approved by the Food and Drug Administration, protects against nine such strains and, researchers say, may be able to prevent up to 90 percent of cervical cancers. Genetic variants are associated with susceptibility to mouth and throat cancer

The most noteworthy finding was an association between cancer of the oropharynx and certain polymorphisms (alternative versions of a given DNA sequence) found in the human leukocyte antigen (HLA) genomic region. HLAs, proteins found on the surface of most cells in the body, play an important role in recognizing potential threats and triggering the immune response to foreign substances. According to Eloiza Helena Tajara, a professor at the São José do Rio Preto Medical School (FAMERP) in São Paulo State, Brazil, and co-author of the article, a specific group of variants in this region, located on chromosome 6, is associated with enhanced protection against oropharyngeal cancer caused by human papilloma virus (HPV). "Previous research showed that these same variants confer protection against cancer of the uterine cervix, which is known to be associated with HPV," Tajara said. "Our findings suggest that the genes that control the immune system play a key role in predisposition to HPV-related tumors. This discovery points to the possibility of clarifying the mechanisms whereby such tumors develop and of designing methods for monitoring risk groups." Chronic sinusitis linked to head and neck cancers in elderly

Source: www.cancernetwork.com Author: Anna Azvolinsky Chronic sinusitis is associated with three rare types of head and neck cancer, including nasopharyngeal cancer, human papillomavirus (HPV)-related oropharyngeal cancer, and nasal cavity and paranasal sinus cancers, according to a new study published in JAMA Otolaryngology–Head & Neck Surgery. Chronic sinusitis is local inflammation caused by either a virus or bacteria that lasts for longer than 12 weeks. Either the chronic inflammation from the sinusitis, the immunodeficiency that can accompany chronic sinusitis, or both may contribute to the development of these head and neck cancers. The effect is modest, however, wrote the study authors. “There are currently no general US guidelines for head and neck cancer screening, but given the low absolute risk, our findings do not support a need for head and neck cancer screening in individuals with chronic sinusitis,” wrote study authors Daniel C. Beachler, PhD, MHS, and Eric A. Engels, MD, MPH, of the infections and immunoepidemiology branch of the National Cancer Institute in Bethesda, Maryland. The absolute risk of these cancer types was low. At 8 years after a chronic sinusitis diagnosis, they had a cumulative incidence of less than 0.07%. HPV vaccine important preventive tool for survivors of childhood cancer

A survey of 679 survivors one to five years out of treatment was conducted to evaluate how many received the vaccine compared with their healthy peers. A great disparity was discovered, as survivors — all of whom were between age 13 and 26 — were less likely to receive the vaccine. Researchers also investigated the reasons why many survivors did not receive it. Klosky, an associate member and director of psychological services in cancer survivorship at St. Jude Children’s Research Hospital, presented the findings from this research at the 2016 Annual Meeting of the American Society of Clinical Oncology (ASCO), a gathering of 30,000 oncology professionals in Chicago. Hepatitis C Virus strongly linked to head and neck cancer: study

Researchers from the Univerity of Texas MD Anderson Cancer Center have revealed that HCV-infected people are at a higher risk of developing these cancers by around two to five times more than those without the Hepatitis C infection. "What we are trying to make all understand is that this is an infection that has consequences – and it's an infection we can cure," said Dr. Harrys Torres, lead author and an associate professor of infectious diseases at the university. Torres further explained that the Hepatitis C infection is easily curable and that over 90 percent of the HCV-related cases can be cured without any trouble simply by taking antiviral drugs, although they are expensive. Also, it is recommended that people should ensure timely screenings and appropriate treatment for HCV, as this can considerably prevent the condition from developing in the body. Hepatitis C appallingly affects more than 3.5 million Americans, according to the U.S. Centers for Disease Control and Prevention. Cancer treatment: New method helps white blood cells fight tumors

The patients, who have head and neck cancer, are receiving genetic modifications that help their white blood cells recognize and attack tumorous growths. Although white blood cells are naturally equipped to eliminate unnecessary and infected cells, they sometimes need help to combat cancer cells. The team of scientists is taking blood samples and treating the white blood cells with a virus that introduces two new genes - the first makes cell growth in the laboratory easier, and the second helps the white blood cells identify and attack tumors. "In most cancers, metastasis, the spread of a disease from the part of the body where it started to another not directly connected, is the commonest cause of death," said John Maher, principal investigator of the trial. "However, head and neck cancer is unusual in that local spread or recurrence of the disease accounts for most suffering and death. This means that tumours may become inoperable and do not shrink in response to traditional treatments such as chemotherapy or radiotherapy." Today, the demographic for the disease includes younger people of both sexes with no history of deleterious social habits who are otherwise healthy and active. It spans all socioeconomic, racial, religious, and societal lines. In other words, oral and oropharyngeal cancer is an equal opportunity killer. Today, as you read this article, 24 people in the US will lose their battles with oral cancer. That is one person for each hour of the day, every day of the year.
