Diet, nutrition, and cancer — don’t trust any single study

Source: American Association for Cancer Research (AACR) 100th Annual Meeting Author: Zosia Chustecka Numerous studies on diet and cancer were presented here at the American Association for Cancer Research (AACR) 100th Annual Meeting, but several of the findings that were highlighted in AACR press releases — and thus are likely to be picked up by the lay media — run counter to the accumulated body of evidence, and some of the comments based on these studies are untrue or premature. So said Walter Willet, MD, DrPH, from the department of nutrition at Harvard School of Public Health, in Boston, Massachusetts, in an exclusive interview with Medscape Oncology. "No conclusions should be made on the basis of a single study," he said. Dr. Willett presented an overview entitled "Diet, Nutrition, and Cancer: The Search for Truth," in which he reviewed many of the associations that have been suggested by epidemiologic studies. These include consumption of red meat, meat cooked at a high temperature, a high-fat diet, and alcohol all increasing the risk, and fruit and vegetables decreasing the risk. However, much of the evidence for these links is rather weak, he said; the most robust evidence supports a link between obesity and an increased risk for cancer. "The estimate that diet contributes to around 30% to 35% of cancers is still reasonable," Dr. Willet said, "but much of this is related to being overweight and inactive." Pretreatment health behaviors predict survival among patients with head and neck squamous cell carcinoma

Purpose: Our prior work has shown that the health behaviors of head and neck cancer patients are interrelated and are associated with quality of life; however, other than smoking, the relationship between health behaviors and survival is unclear. Patients and Methods: A prospective cohort study was conducted to determine the relationship between five pretreatment health behaviors (smoking, alcohol, diet, physical activity, and sleep) and all-cause survival among 504 head and neck cancer patients. Results: Smoking status was the strongest predictor of survival, with both current smokers (hazard ratio [HR] = 2.4; 95% CI, 1.3 to 4.4) and former smokers (HR = 2.0; 95% CI, 1.2 to 3.5) showing significant associations with poor survival. Problem drinking was associated with survival in the univariate analysis (HR = 1.4; 95% CI, 1.0 to 2.0) but lost significance when controlling for other factors. Low fruit intake was negatively associated with survival in the univariate analysis only (HR = 1.6; 95% CI, 1.1 to 2.1), whereas vegetable intake was not significant in either univariate or multivariate analyses. Although physical activity was associated with survival in the univariate analysis (HR = 0.95; 95% CI, 0.93 to 0.97), it was not significant in the multivariate model. Sleep was not significantly associated with survival in either univariate or multivariate analysis. Landmarks in the History of Cancer Epidemiology

Dunn The application of epidemiology to cancer prevention is relatively new, although observations of the potential causes of cancer have been reported for more than 2,000 years. Cancer was generally considered incurable until the late 19th century. Only with a refined understanding of the nature of cancer and strategies for cancer treatment could a systematic approach to cancer prevention emerge. The 20th century saw the elucidation of clues to cancer causation from observed associations with population exposures to tobacco, diet, environmental chemicals, and other exogenous factors. With repeated confirmation of such associations, researchers entertained for the first time the possibility that cancer, like many of the infectious diseases of the time, might be prevented. By the mid-20th century, with antibiotics successfully addressing the majority of infectious diseases and high blood pressure treatment beginning to affect the prevalence of heart disease in a favorable direction, the focus of much of epidemiology shifted to cancer. The early emphasis was on exploring, in greater depth, the environmental, dietary, hormonal, and other exogenous exposures for their potential associations with increased cancer risk. Calcium tied to lower cancer risk in older people

The benefits were mostly associated with foods high in calcium, rather than calcium tablets. Previous studies have produced conflicting results. The new research involved food questionnaires from participants and a follow-up check of records for cancer cases during the subsequent seven years. This research method is less rigorous than some previous but smaller studies. But because of its huge size — 492,810 people and more than 50,000 cancers — the new study presents powerful evidence favoring the idea that calcium may somehow keep cells from becoming cancerous, said University of North Carolina nutrition expert John Anderson, who was not involved in the study. The study was run jointly by the National Institutes of Health and AARP. The results appear in Monday's Archives of Internal Medicine. National Cancer Institute researcher Yikyung Park, the study's lead author, called the results strong but said more studies are needed to confirm the findings. Duke University nutrition researcher Denise Snyder said the results support the idea that food rather than supplements is the best source for nutrients. Dietary factors and oral and pharyngeal cancer risk

Source: Oral Oncol, November 4, 2 Authors: Ersilia Lucenteforte et al We reviewed data from six cohort studies and approximately 40 case-control studies on the relation between selected aspects of diet and the risk of oral and pharyngeal cancer. Fruit and vegetables were inversely related to the risk: the pooled relative risk (RR) for high vegetable consumption was 0.65 from three cohort studies on upper aerodigestive tract cancers and 0.52 from 18 case-control studies of oral and pharyngeal cancer; corresponding RRs for high fruit consumption were 0.78 and 0.55. beta-carotene, vitamin C and selected flavonoids have been inversely related to the risk, but it is difficult to disentangle their potential effect from that of fruit and vegetables. Whole grain, but not refined grain, intake was also favorably related to oral cancer risk. The results were not consistent with reference to other foods beverages, and nutrients, but it is now possible to exclude a strong relation between these foods and oral and pharyngeal cancer risk. In western countries, selected aspects of diet may account for 20-25% of oral and pharyngeal cancer, and the population attributable risk increases to 85-95% when tobacco and alcohol consumption are also considered.
